Position Description:

The Towards Change Team Leader role makes up part of the Towards Change Team and is a crucial part of the Victorian Mental health Reform work that APMHA and its partners are undertaking. This role will be part of a growing service system which places the community in the centre of the Local Mental Health and Wellbeing (Local) network across Victoria.

The Towards Change Team provide a range of assessments depending on client need and will provide evidence-based interventions beyond mental health, extending to physical health, substance dependence issues, employment and relationships support, as part of the broader Local workforce.

You will lead the Towards Change Team as they provide assessment and care planning for clients referred via the Support and Connect Program, working alongside dedicated Wellbeing Peer Navigators. The role will have overall responsibility for managing the Towards Change Team members as they deliver specialist, evidence based clinical interventions for participants, as part of the broader Local support team.

Candidates will need:

  • Tertiary qualifications in a relevant field
  • Experience working in mental health/AOD (minimum 3 years)
  • Experience in a Team/Senior Leader or Management role
  • Demonstrated time management abilities, excellent communication (written and oral) and interpersonal skills.
  • Demonstrate a friendly, proactive approach and work with competing needs, deadlines and balance priorities with efficiency and professionalism.
  • Ability to work independently and within a team.
  • Certification of qualifications

The Towards Change Team are responsible for providing assessment, counselling and care planning for clients referred via the Support and Connect Program, and will work alongside dedicated Wellbeing Peer Navigators from the Support and Connect Team

The Towards Change Team clinician, employed by APMHA and working with Peer Navigators from the Support and Connect Team, will provide evidence based clinical interventions for participants. The Towards Change Team will encompass clinically and therapeutically proficient clinicians, as well as a Creative Arts Peer Worker, providing tailored mental health support to clients and their families via their wellbeing plan, including to those with co-occurring substance use or addiction.

Candidates will need:

  • Current AHPRA Registration for Mental Health Nurses, Psychologists and Occupational Therapists OR Registration with AASW for Social Workers
  • Post graduate experience working in a mental health or AOD setting (minimum 6 months)
  • Demonstrated time management abilities, excellent communication (written and oral) and interpersonal skills.
  • Demonstrate a friendly, proactive approach and ability to work with competing demands and balance priorities with efficiency and professionalism.

Location: Residential Aged Care Facilities across Brimbank, Hobsons Bay, Maribyrnong, Melton, Moorabool, Wyndham, Moonee Valley

Remuneration – Employee:
